Use this form if your organization will be hosting an event and/or meeting at which food will be served. Be as specific as possible when filling out the Food Waiver Form.
The Food Waiver Form must be submitted to the Assistant Director of Student Engagement at least two (2) weeks prior to the meeting/event.
Complete this form for ANY events or meetings your club is hosting. Please have Event Reference Number as provided by 25Live/CollegeNet (2017-ABCDEFG) ready in order to submit.
Failure to submit your event and/or meeting may jeopardize your club’s recognition with the Smeal College of Business.
